If you are interested in coming to Morningside Nursing and Rehabilitation Center, our admissions team is here to help you get started. They will guide you through placement in our sub-acute care, rehabilitation, and skilled nursing programs, and collaborate with your social worker to ensure a smooth transition back to your home and community. Our team will help you understand our policies and procedures, address any questions, and ensure you feel comfortable with every aspect of the process. Additionally, we can arrange a tour of the facility at your convenience.
Eligibility
We welcome all candidates aged 18 and over who qualify for nursing home placement, sub-acute care, or rehabilitation. Our admissions team will work with you to confirm your suitability for our programs and will require a completed application, a Patient Review Instrument, and a Screen. If you are interested in Medicaid coverage, our Medicaid Coordinator can assist you in determining your eligibility and guide you through the application process.
